Application Procedures

Procedure for Referral
Assessments for placement at E.S. le Grange School, are done by the Occupational Therapist, Physiotherapist  and the Educational Psychologist.
 
Step 1
The School-Based Support Team coordinator of the learner’s current school phones E.S. le Grange School to make an appointment.
The following information of the learner is needed:
  • Name and Surname
  • Date of Birth
  • Current School
  • 2 contact numbers
The Secretary will phone back to schedule an appointment for an assessment to be done by one of the Therapists or the Educational Psychologist.
 
Step 2
An assessment is done to determine if the assessed learner falls within the criteria for admission to E.S. le Grange School.  
The assessments were compiled in such a way that a broad set of skills can be tested to get a holistic picture of the child.
The following forms must be brought along to the appointment:
  • Copy of birth certificate of learner
  • Copy of Identity Document of parent
  • Copy of learner’s clinic card
  • Copy of any medical reports
  • Forms provided by the learner’s current school (if they are attending a school)
    • Copy of Support Needs Assessment forms (SNA 1 and 2 ) and DBE 120 - completed by the School-Based Support Team of the learner’s current school
    • Copy of learner profile 
    • Copy of school reports
 
Step 3
If the learner is a suitable candidate for E.S. le Grange School, the parent will receive other documents to fill in. Completed forms must be submitted as soon as possible.
If the learner is not a suitable candidate for E.S. le Grange School, he/she will be referred to a suitable school/ care center if possible.


Aansoekprosedures

Prosedures vir verwysing
Assesserings vir plasing by E.S. le Grange-skool, word gedoen deur die Arbeidsterapeut, Fisioterapeut en die Opvoedkundige Sielkundige.

Stap 1
Die skoolgebaseerde ondersteuningspan-koördineerder van die leerder se huidige skool telefoneer E.S. le Grange Skool om 'n afspraak te maak.
Die volgende inligting van die leerder word benodig:
  • Naam en van
  • Geboortedatum
  • Huidige skool
  • 2 kontaknommers
Die sekretaresse skakel terug om 'n afspraak te maak vir 'n assessering wat deur een van die terapeute of die opvoedkundige sielkundige gedoen moet word.

Stap 2
'n Assessering word gedoen om vas te stel of die leerder binne die kriteria vir toelating tot E.S. le Grange Skool val.
Die assesserings is so saamgestel dat 'n breë stel vaardighede getoets kan word om 'n geheelbeeld van die kind te kry.
Die volgende vorms moet saam na die afspraak gebring word:
  • Afskrif van die geboortesertifikaat van die leerder
  • Afskrif van identiteitsdokument van ouer
  • Afskrif van die leerder se kliniekkaart
  • Afskrif van enige mediese verslae
  • Vorms wat deur die leerder se huidige skool verskaf word (as hulle 'n skool besoek)
  • Afskrif van assesseringsvorms vir ondersteuningsbehoeftes (SNA 1 en 2) en DBE 120 - voltooi deur die skoolgebaseerde ondersteuningspan van die huidige skool
  • Afskrif van die leerderprofiel
  • Afskrif van skoolverslae

Stap 3
As die leerder 'n geskikte kandidaat is vir E.S. le Grange-skool, sal die ouer ander dokumente ontvang om in te vul. Voltooide vorms moet so spoedig moontlik ingedien word.
As die leerder nie 'n geskikte kandidaat vir E.S. is nie, sal hy / sy indien moontlik na 'n geskikte skool / sorgsentrum verwys word.
